Send documents your daycare asked for
When your daycare requests a birth certificate, immunization record, or a filled-out form, scan the paper with your phone or upload a file, then submit.
Sometimes your daycare needs a copy of something you already have: a birth certificate, an immunization record, a physical the doctor signed. They send you a document request, a short checklist of what they need. You get an email, and the request appears in your Documents list under Action needed.

Open the request
- Open the email from your daycare and follow the link, or sign in to NestliCare directly.
- On the Documents page, look in the Action needed group at the top.
- Open the request. You will see one card per document they asked for, each marked Required or Optional.
Two ways to add a document
Each card offers two choices.
Scan paper (in the NestliCare app on your phone). Best for anything printed. Point your camera at the page and the app finds the edges and straightens it. Take every page of the document one after another, then tap Save. All the pages are combined into a single PDF and added to the card. If the document is long, you do not have to scan it in one go: tap Scan more and the next scan is added as another file.
Upload a file. Best for something already on your phone or computer, such as a photo someone texted you or a PDF the doctor's office emailed. PDFs and photos (JPG, PNG) are all fine.
You can add more than one file to a card, up to ten. Tap x or Remove next to a file to take it off before you submit.
If your daycare attached the blank form
Some requests come with the form itself, for example a state health form that a doctor has to complete, or an agreement your daycare wants filled in by hand. You will see a Need the form itself? row at the top of the request.
- Tap that row to view the blank form.
- Tap the share or open button at the top to print it or save it. Print at home, or take it to your appointment.
- Fill it out by hand, or have the doctor fill it out.
- Come back to the request and tap Scan paper on the matching card. Scan every page, then tap Save.
That is the whole round trip: download, print, fill out, scan, submit.
Submit
Once every Required card has at least one file, the Submit documents button turns on. Tap it and you are done. Your daycare is notified and reviews what you sent.
If something is missing or hard to read, your daycare can send the request back with a note. It reappears under Action needed so you can add or replace files.
Tips for a clear scan
- Lay the page flat on a plain surface with good light. A dark table behind a white page helps the app find the edges.
- Keep the whole page in frame. The app crops to the page, so a little background around it is fine.
- Check the preview before you save. You can retake a page if it is blurry.
- Scan the back of a page too if anything is printed there.
Troubleshooting
I do not see Scan paper. Update the NestliCare app to the latest version. On a device without a camera the app shows only Upload a file.
The upload says the file is too big. PDFs must be under 20 MB and photos under 10 MB. Scans made in the app are already compressed and stay well under that. For a photo from your camera roll, take a screenshot of it or share it at a smaller size.
I submitted the wrong file. Ask your daycare to return the request. Once they do, it comes back to Action needed and you can replace the file.
