Fill out a form from your daycare

Fill in a form your daycare sent you, draw your signature once and apply it to every line, check it over, and submit.

When your daycare needs paperwork from you, an enrollment form, medical info, a photo release, they assign a form to your family. You get an email, and the form appears in your Documents list to fill and sign.

You can do this in a browser or in the NestliCare app on your phone. It is the same form and the same draft either way, so you can start on your phone and finish on a laptop.

Open your form

  1. Open the email from your daycare and follow the link, or sign in to NestliCare directly.
  2. On the Documents page, look in the Action needed group at the top.
  3. Tap Fill out.

Your daycare's actual paperwork opens on screen, with the fields sitting on the document where the blanks are.

Fill in the fields

  • Text fields: type directly.
  • Dates: pick from the calendar or type it.
  • Checkboxes: tap to tick, tap again to untick.
  • Dropdowns and radio buttons: pick an option.
  • Photo upload: take a photo or choose a file.

Your work saves itself every couple of seconds. You can close the page and come back without losing anything.

Fields that are already filled in

Some fields arrive pre-filled from what your daycare already has: your name, your child's name, your phone number. They have a soft yellow tint.

You can type over any of them. If your phone number or address has changed, correct it here.

For the fields that matter most, like allergies and emergency contacts, your daycare can require you to look at each one. If so, a banner at the top lists them and submitting stays locked until you have been through them. Tapping a field is enough to confirm it.

Sign it

Signatures now go on the document, where the signature line is.

  1. Tap a signature line on the form.
  2. Draw your signature with your finger, a stylus, or the mouse. Use Clear to start over.
  3. Type your full legal name, tick the agreement, and choose Adopt and sign.

That is the only time you draw. Every other signature line on the form, and a long enrollment packet can have a dozen, is then a single tap to apply the same signature. One person signing thirteen times is one signature, not thirteen different marks.

Check it over, then submit

When every required field is filled, you move to a review of the finished document. It is the real form, read-only, with your answers exactly where they will appear on the signed copy. This is your chance to catch a wrong date before it becomes a PDF.

From review, tap Submit. If you signed by typing your name rather than drawing, you get one more dialog to enter it, and then submit.

The form moves to Awaiting review. Your daycare either approves it, or sends it back with a note if something needs changing.

If your daycare sends it back

You get an email with their note, the form returns to Action needed, and the note appears at the top when you reopen it. Fix the fields they flagged and submit again. Everything you did before is kept.

If the note leaves you with a question, reply to that email: it reaches the staff member who reviewed your form.

Find your signed copy

Once approved, the form moves to Completed on the Documents page. Open it and use Download signed PDF to save your stamped copy.

Troubleshooting

I cannot find the form. Check you are signed in as the parent it was assigned to. If your family has two adults, it may have gone to the other one.

Submit is greyed out. Either a required field is empty, a signature line is unsigned, or there are fields the banner is asking you to check. The button that jumps you to the next outstanding item is the fastest way through.

I came back to a form I had already signed and could not submit it. That was a bug and it is fixed. If you still see it, redraw the signature once and submit.

"Could not load PDF". Reload the page. If it keeps happening, message your daycare; they may need to reassign the form.

Drawing does not work on my device. Type your name instead. A typed name is a legally equivalent signature and your daycare receives the same signed PDF.

I submitted before I was finished. Message your daycare. They can send it back to you, and you can edit and resubmit.
