Submit an enrollment packet
Fill out the forms your daycare sent you — emergency contacts, medical, consent forms — via the email link.
When you first enroll at a new daycare, the center may send you an enrollment packet. It's a bundle of forms: emergency contact info, immunization records, photo release, dietary restrictions, and whatever else your state or center requires.
You'll fill it out on the web, via the email link
Enrollment packets are web-only for now. You'll get an email with a secure link. Tapping the link opens a browser page with your packet. You don't need a NestliCare account to fill it out — the link is unique to you.
Open your packet
- Check your email for a message from your daycare. The subject usually includes your center's name (e.g., "Sunny Meadows — Enrollment Forms").
- Tap the Open your packet button or link.
- A browser tab opens to your personalized packet.
You'll see a list of forms to complete. Each one has a status: not started, in progress, or submitted.
Fill out a form
Click a form in the list. Form types vary:
- Fill-in fields — names, addresses, dates, emergency contacts. Type directly in the fields.
- Yes/no questions — e.g., "Does your child have any allergies?" Click a radio button.
- Longer text — notes, special instructions.
- Date pickers — for date of birth, immunization dates, etc.
- Signature — draw with your finger on mobile, or your mouse on a computer. Or type your name (legal equivalent for this kind of form).
- File upload — for immunization records, custody documents, etc. PDF, JPG, PNG, or HEIC, up to 10 MB per file.
Click Save & Continue to move to the next form. Your progress is saved as you go — you can close the tab and come back via the same email link later.
Submit everything
When every form shows Submitted, you're done. The daycare gets a notification and reviews your submission.
If any form is marked Rejected after review — usually a missing field or incorrect info — the daycare sends a follow-up email with a note on what to fix. Click your original link again, edit the flagged form, and resubmit.
What the center sees
On the admin side, your submission appears in their enrollment packet review queue. They can:
- Approve — everything looks good, you're in
- Reject with a note — they need you to fix something
- Ask for more info via Messages
Typical review time is 1-2 business days.
Common packet contents
Depends on your state, but usually includes:
- Emergency contact form — 2-3 adults beyond the parents who can be called
- Medical form — allergies, chronic conditions, medications, doctor info
- Immunization record — either typed dates or a photo of the pediatrician's card
- Photo release — whether your child's photos can appear in center communications
- Consent to administer medication — permission for staff to give tylenol, sunscreen, etc.
- Parent handbook acknowledgment — you read it, you agree
- Tuition agreement — schedule, rates, late-pickup fees, cancellation policy
Your center picks what to include based on state licensing requirements.
Tips for a smooth submission
- Have your child's pediatrician info handy — name, phone, address
- Scan immunization records ahead of time — or photograph the original card in good lighting
- Use a real email address — packet links are scoped to your email; no alternates
- Save as you go — the system saves each form on next-click, but if you're on a poor connection, save more often by hitting Save & Continue
- Use a computer if possible — forms with long text work better with a real keyboard
Troubleshooting
Email never arrived. Check your spam folder. Then ask your daycare to resend — they can from their admin side.
Link says "expired." Packet links don't expire, but if the daycare revoked and re-sent, the old link stops working. Check your inbox for a newer email.
"File too large" on upload. 10 MB per file. Compress the image (most phones have a "reduce size" option in the share sheet) or export a PDF at lower DPI.
Signature keeps looking blank. Some mobile browsers don't capture drawn signatures well. Use the Type signature option — typing your full name is legally equivalent for these forms.
Submitted a form with wrong info. Re-open the packet link. Click the form. Edit and submit again.
I don't see one of the forms my daycare said would be there. The packet they sent might not include that form. Message them from inside NestliCare (once you're enrolled) or reply to the packet email with a question.
